Your complimentary 60-minute new patient orthodontic evaluation will consist of:
- Orthodontic Evaluation
- Diagnostic Records (includes a digital panoramic image and photos)
- Potential Treatment Options
We are committed to educating our patients each step of the way, providing a clear understanding of our proposed treatment, along with addressing any questions or concerns you may have. You will also go home with a personalized treatment plan outlining our consultation exam with you.
We ask that you bring the following to this visit:
- A completed Health History Form (we will mail this to you upon scheduling your appointment, or we may ask you to print off a form from our website)
- Insurance information (we'll be happy to verify coverage and available benefits upon your arrival)
